Challenges
associated with aging are rarely the result of one issue; more often
they are complex and inter-related! In order to plan for effective
solutions, multiple assessments and interviews must be conducted by
qualified professionals. By completing an individually tailored
combination of the assessments listed below, according to need,
effective solutions can be identified and recommended, relieving the
stress of families and professionals and providing expert guidance.
The assessment process, conducted by our Master's level
gerontologists, geriatric care managers, RN’s, and other team members,
may include any combination of the following:

Once the assessment data is gathered, a professional case
conference is held, and the information is compiled and analyzed by our
professional inter-disciplinary team. Problem areas are outlined and
inter-related, with potential solutions that are tailored to the unique
circumstances of the aging individual and family.
Finally,
based upon our findings, a written Life Care Plan is
presented, with immediate and long term recommendations for the aging
individuals' quality of life,reviewed in a final case conference with
the family, physicians, and client, as desired. |
